Full Job Description
- To provide strategic and operational management to the Home 1st Community Therapy Teams: Admission Avoidance, Community Therapy, Staying Steady and Pulmonary Rehabilitation
- To provide transformational, integrated leadership and service modernisation to the teams
- To enhance the development of evidence based clinical practice and Allied Health Professional roles
- To provide clinical and professional leadership to the team
- To be aware of the national and local agendas for intermediate care services and Allied Health Professions
- To work alongside and in conjunction with the other Home 1st services to ensure the safety and wellbeing of the patients.
- To work with the service manager and the wider health and social care economy to develop strategies in line with the National Agenda and in particular around admission avoidance and supported discharge
- To have oversight of the work of The Care Academy.
- To work with the AHP Professional Leads for CNWL to develop Therapy roles within CNWL and promote the professions

Home 1st is a multi-disciplinary integrated intermediate care service, which encompasses collaborative working by health and social care, along with other local providers, to support the physical therapy needs of the people of Milton Keynes. Providing admission avoidance, supported discharge and specialist therapy pathways, this role will require organisational leadership at a dynamic pace., In this leadership role, we can offer career coaching, opportunities for mentorship and career development via leadership training. You will have access to 1:1's, appraisal and peer networking. Home1st is based in Bletchley and the modern office is a central hub for multi-disciplinary working.
