Full Job Description
We seek a Band 4 Technical Instructor to join our friendly team. You'll be making a real difference to your patients every single day - delivering outstanding community care as a valued member of the Discharge Home to Assess Team.

Main duties of the job
We are looking for full time applications.
The service runs 8am to 6pm Monday to Friday and 9am to 5pm at weekends. The post holder will be expected to contribute to the weekend working Rota.
The service covers both North and East Hertfordshire and the post holder may be expected at times to cover across localities.
You must hold a full valid driving licence and have access to a car to use regularly for business purposes are essential (unless you have a disability as defined by the Equality Act 2010).

You need to apply before the interview takes place., Working within a multi-disciplinary team of health and social care professionals, providing support for clinical care to patients closer to home as part of the patient pathway from the acute sector through to discharge.
To provide support, assessment and treatment for patients within their own homes providing appropriate aspects of exercise, activity and treatment programmes which may be prescribed by a clinician, with the aim of rehabilitation and optimising potential.
To carry out non-clinical activities which facilitate the efficient running of the team (e.g. clerical support).
