From 4 April 2024 overseas workers requiring a general Skilled Worker Visa (such as data analysts, IT and finance professionals, etc) but sponsored by an NHS organisation will need to meet the new salary threshold of £38,700. In addition, applications to the Health and Care Visa route for those occupations paid by NHS bands that are on the immigration salary, a list of skilled jobs which have lower salary requirements, will need to meet the national pay scale for their role or the new minimum salary threshold of £23,200 (whichever is higher). The position are an ideal opportunity for Physiotherapist or Occupational Therapist, to shape this new service for our patients. Are you passionate about supporting patients to discharge home as soon as they are able to. You will be working and leading a team that is motivated to ensure that patients should be returning home as soon as they are able. Ethos of the team is 'Why not home, why not today'
Shropshire Community Health Trust is offering an exciting opportunity for motivated individuals to work within the Care Transfer Hub at the Shrewsbury and Telford Hospital Trust. The team is expanding and this is an ideal opportunity to be involved in the transformation of the service and work with the existing skilled team in the specialist field of Complex Discharge.
The successful applicants will be key members of the Care Transfer Hub with an multidisciplinary of health and social care teams.
Please note this vacancy may close early if a high volume of applications is received, we advise you submit your application early to avoid disappointment., As a senior clinician within the Care Transfer Hub (CTH) multi-disciplinary team you will work with a "home first" philosophy. You will be an advocate for the patient in determining the right pathway for them as individuals following a discharge to assess model. You will be working in collaboration with other health and social care professionals in an integrated way
providing autonomous clinical, professional decision-making to support the planning of complex discharges. The post holder will develop the ethos of multi-disciplinary and seamless services within Shropshire Telford and Wrekin's acute and or community hospitals ensuring that discharge planning is delivered to the highest possible standard.
The post holder will be working flexibly across both acute hospital sites and may be required to work in community settings as part of their role and participate in 7 day working when appropriate

The position is an ideal opportunity to be part of shaping the service which is at the forefront of the NHS long term plan agenda. You will be instrumental with the most complex needs from the hospital setting and reviewing the discharge pathway and services including home and bed based reablement services, urgent community response (UCR) services including admission avoidance /Virtual Wards and community teams.
