Full Job Description

We are seeking a Band 4 Therapy Assistant Practitioner with a passion for hospital avoidance & community rehabilitation in peoples own homes.

The successful candidate will share in our vision of the 'home first' approach. You will have the opportunity to work in a large MDT to deliver a 2-hour response as part of the National Urgent Care Response targets. Our aim is to reduce unnecessary hospital admissions and prevent avoidable Emergency Department attendances by stabilising patients within the place they call home.

The OBI team is a therapy service providing assessment and treatment to complex patients. The role will involve crisis response work within a patient's own home, and short-term rehabilitation. You will have experience in delivering therapeutic programmes of care in a patient's home, including, but not exclusively: exercise programmes, provision of equipment and enabling activities of personal and domestic daily living tasks. Knowledge of community services and an understanding of community urgent response pathways would be advantageous.

Within the team we have Occupational Therapists, physiotherapists, nurses and medics. We work as part of a collaborative MDT to provide rapid assessment and intervention to people across the Dudley borough.

The successful candidate will be able to work autonomously in the community but will have support from senior staff., To provide effective and efficient rehabilitation under the supervision of qualified therapists to a caseload of adult patients with a variety of conditions who require community rehabilitation.

To carry out and progress rehabilitation programs set by Physiotherapists and Occupational Therapists, working alone in patient's homes.

To carry out non-complex assessments within scope of practice as required meet the needs of the patient.
