Full Job Description

The Adult Ability Team is a unique service, supporting patients with Progressive Neurological Conditions with a friendly, highly skilled motivated multidisciplinary team, which is fully integrated into the wider Community Therapy Teams in Burton-on-Trent, East Staffordshire. To support the development and growth of this team we are looking for an enthusiastic and passionate Band 4 Assistant Practitioner with an interest in aligning their skills working mainly with patients with Occupational Therapy needs, to join us to make a difference to people's lives within our community. You willalso work alongside Physiotherapists, Nurses and support workers in this evolving team to support home based assessments and the implementation of treatment plans for neurological patients in the community. This team has a 100% supervision recording and works seamlessly with Rapid Response, Community Rehab and Falls Teams and D2A services, and has well-established links with the Acute Sector and Primary Care., As an Assistant Practitioner joining our team, you will bring advanced knowledge, skills and a creative approach to support the development and growth of this team to create a positive impact and change patient outcomes for the better.

The ability to travel independently across East Staffs is essential for this role.
