Full Job Description

The Band 4 TAP post is an integral role within the therapy team and, by working closely with Physiotherapists, Occupational Therapists and discharge managers, they will help to ensure an optimum and seamless patient experience for our patients. It may involve travelling to other sites.

The successful post holder will work within the emergency department supporting safe discharges back into the community. You will be responsible for communicating with the community teams to ensure that all patient's discharged have the most appropriate follow-up care and intervention as required.

Main duties of the job

Carrying their own clinical caseload of patients within the emergency floor without direct supervision but with guidance from the AHPs within the team.

Keeping the people who are using our services as safe as possible by recognising the limits of the Assistant Practitioner role and working closely with the registered clinician.

Contributing to the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based policies and procedures when implementing treatment / management plans.

Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people when contributing to the implementation and evaluation of treatment plans.

To ensure high quality documentation and appropriate and timely referrals

Working closely with the whole MDT to provide seamless patient care.

Ensuring they maintain their own continued professional development and participate in any training provided

To support with safe and timely discharges across the emergency floor making onward referrals as necessary.
