Full Job Description
This position rotates between the community therapy team (working with patients in their own homes) and the 'in-reach' rehabilitation beds team (supporting rehabilitation and discharge planning in x3 care homes) . You will work alongside Physiotherapists' and Occupational therapists to support carrying out exercise programme's and functional activities to support patients with their rehabilitation. This post involves working with patients with a wide variety of pathologies and medical conditions, who have recently been discharged from hospital and require additional time to help improve their mobility and independence before returning home. Due to the domiciliary nature of this post a full UK driving license and access to a car is desirable. The post is subject to successful interview, references, DBS clearance and Occupational Health checks. We are looking for enthusiastic individuals who want to work with people and provide personalised care, so whilst we welcome applicants who have an NVQ in healthcare, not having a recognised qualification is not a barrier to applying. - Access to a wide range of benefits and discounts for NHS staff 1. a) The post holder will work with a supported and supervised caseload. 2. b) To carry out delegated tasks as part of a treatment programme of monitoring and reviewing progress 1. c) To provide treatment programme's to clients on an individual or group basis. 2. d) To report and document variances in a patient's condition or planned pathway of care. 1. e) To participate in skill sharing and acquisition of clinical competencies, in line with the competency frameworks. 1. f) To maintain high standards of clinical care as defined by the Sussex Community NHS Foundation Trust Communication and Working Relationships 1. a) To build professional rapport with patients and their families, communicate with them in a way that respects their views, autonomy and culture, ensuring that they are involved in and informed about their care. 1. b) To use effective communication skills to encourage patients to participate in treatment, adapting approach to meet diverse needs of client group, seeking guidance from colleagues where necessary. 1. c) To maintain confidentiality and dignity at all times and ensure people's equality and diversity needs are supported. 1. d) To communicate effectively and work collaboratively with other members of the team regarding patients' status, reporting back to the registered clinician, providing verbal or written feedback as appropriate. 1. e) To report effectively to the team/ registered staff member on patient's progress and performance in relation to the patient goals. 1. f) To attend patient-related multi-disciplinary meetings as directed by registered staff. 1. g) To maintain accurate, comprehensive and up-to-date patient-related documentation including electronic patient records, where appropriate.
