Full Job Description

We are seeking to recruit an experienced, competent, and motivated Therapy Senior Support Worker to work within the Cardiac Rehabilitation Service in Doncaster.

The cardiac rehabilitation service currently comprises of nurses and physiotherapy staff who offer comprehensive assessment and treatment to adults in Doncaster who have had a recent cardiac event, for example heart attack, cardiac surgery and other procedures, heart failure diagnosis and arrythmia.

The aim of cardiac rehabilitation is to offer an individualised plan to patients in order to support them in better understanding their condition through education around lifestyle, risk factor management including exercise, diet, weight management, smoking cessation, medication management, symptom control, to support with their psychosocial health, to prevent worsening of their condition, reduce the risk of future heart problems and to improve quality of life. This is done via an integrated team approach and this role offers an exciting opportunity to take part in all of the components of cardiac rehabilitation with patients, with a particular focus on the exercise component. You will be a knowledgeable and forward-thinking individual who is able to provide prescribed structured education, exercise, support and guidance to patients.

This is a community-based service and includes clinic settings, home visits and leisure centres including group exercise sessions.

  • To work independently delivering agreed activities with patients / service users with access to senior staff for support when needed.]

  • Supervision of patients whilst in group sessions or 1:1 sessions

  • Review and deliver exercise programmes - mobility/ strengthening and cardio programmes within both group and 1:1 environment

  • Review and progress mobility programmes, assessing for and reviewing with equipment.

  • Deliver and progress relaxation techniques and adapted Tai Chi sessions.

  • Deliver patient education both within group sessions and 1:1

  • To contribute to the running and organisation of a group session

  • Review of patients using standardised therapy assessments where indicated.

  • To report progress of the patient and changes in condition to the appropriate person.

  • To ensure accurate and timely documentation of care on SystmOne

  • To develop clinical competency and skills through the completion of competency-based training, and to maintain skills through regular re-assessment and training.

  • To demonstrate sensitivity to the patient and carer's needs by respecting privacy and dignity, whilst always maintaining professional boundaries.

  • Develop a rapport with patients and encourage them to participate in the programmes and activities
