Full Job Description

  • To manage own caseload on the ward (with support from the qualified therapist as appropriate)
  • To construct, implement, monitor, evaluate and update therapy careplans / treatment programmes
  • To support ward staff with careplans / treatment programmes promoting ongoing rehabilitation.
  • To work independently and as a lone worker in a range of settings including ward settings.
  • To independently carry out and record assessments of an individual's function. Liaising with supervising therapists and other MDT members when appropriate.
  • Provide relevant intervention using a combination of techniques, health promotion, equipment and adaptations to maintain, restore and promote independence in the community, taking into account social, cultural and economic needs.
  • To carry out and deliver care plans and treat patients individually or in a group in a variety of settings without the presence of a therapist.
  • To effectively communicate with clients, family / carer who may have barriers to understanding or communicating e.g. blind, deaf, or who do not have English as a first language.
  • To undertake appropriate communication with members of the multi-disciplinary team, outside agencies and others i.e. clients, carers, relatives etc, community based colleague.

As a result of the opening of Midland Metropolitan University Hospital, Walsall Healthcare NHS Trust are seeking staff to continue to support the Trust's journey to be a leading provider of care for our community., Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor Hospital houses the full range of district general hospital services under one roof. The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children's Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. This is scheduled for completion this year.
  • We provide high quality, friendly and effective community health services from some 60 sites including Health Centres and GP surgeries. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.
