Full Job Description

This exciting role will support the Frailty team to deliver Therapy input to a wide range of patients over the age of 70, with the aim of preventing any unnecessary lengthy hospital stays by working closely with our community teams to support discharges. It will involve a lot of patient contact as well as some administerial jobs to help maintain an optimum patient journey., To undertake specific skilled support work to aid both physiotherapists and occupational therapists in the assessment of patients.

To accept patients delegated by a therapist and manage delegated case load on a day to day basis.

To treat patients according to planned programmes and agreed treatment protocols as individuals or in groups.

To assist in drawing up the treatment programmes in conjunction with a therapist.

To monitor patient progression and make minor alterations to patient treatment programmes, consulting with the supervising therapist.

To report back on patient progress and inform the therapist of any problems.

To assist with administerial duties as required to support the team.

To take an active role in ordering equipment for patient discharge.

To support across the site as needs/workload require
