Full Job Description

This role is based within the acute trust offering cross cover across two sites of around 1337 beds and some outpatient departments. Patients tend to be of mixed ages across all specialities, ranging from neonates to elderly care.
The Tissue Viability Nurse is required to have a sound knowledge of wound care and wound aetiology to facilitate assessment and management of complex wounds.
Assist the Senior Clinical Tissue Viability specialist with the day to day running of the Tissue viability service., This role includes delivery of ward based education and the identification of hospital acquired pressure ulcers with recognition of lapses, lead on the management of pressure ulcer prevention across the organisation including where necessary the management of incontinence where this impacts of skin integrity.
Assess, plan and implement care for patients with acute/chronic wounds.
The Tissue Viability Nurse is required to support/lead on product evaluations and the production and implementation of new pathways/guidelines, + Develop and implement educational programmes of continuous quality improvement within the fields of Tissue Viability including defining risk, and the evaluation of effectiveness of these guidelines and protocols.
+ Undertake presentations on clinical education on a one-to-one, small group or larger group basis. Participate in working groups as required
+ Demonstrates good documentation in relation to clinical care, interventions and treatment.
+ Demonstrates good verbal communication with ward teams relating to individual
+ patients treatment plans and advice.
+ · The post holder will be a registered nurse.
+ The post holder will have a sound understanding of wound healing and the issues that may prevent wounds from healing.
+ The post holder will have a sound understanding of pressure ulcer prevention and management in the acute care setting.
+ The post holder will have wound management experience including active participation in tissue viability link groups/networks.
+ Assist in the assessment of clinical practice in the area of tissue viability across the organisation.
+ Undertake ongoing wound assessment taking into account multiple aetiologies and using current evidence.
+ Collate statistical data relevant to Tissue viability
+ Work will be supervised by the Senior Sister for Tissue Viability
+ The post holder will prioritise and plan their individual workload as allocated by the triaging Tissue Viability Nurse.
+ The post holder will instruct the plan of care for individual patients in relation to their
+ wound management plan, in consultation with the Tissue Viability Senior Nurse.
+ Standard keyboard skills, use of presentation, projection/multi-media equipment and e learning resources.
+ To have dexterity and accuracy requires for the procedures such as dressing complex and extensive wounds.
+ Able to move between several clinical areas on different hospital sites throughout the working day.
+ Able to travel independently to educational venues outside of the Trust as required.
+ To facilitate individualised patient care based on the principles of evidence based practice, in consultation with the Tissue Viability Senior Nurse Autonomous in their decision making and will ensure patient receive a thorough assessment and personalised treatment plan
+ Participate in clinical practice development projects
+ Provide educational support in line with Tissue Viability guidelines.
+ Drives the delivery of education to support staff in the adoption of new policies and procedures.
+ Contributes to the development of policies and procedures as required Develop policies and procedures
+ Demonstrates safe working practice of equipment in the clinical area.
+ Demonstrates accurate and safe allocation of tissue viability related equipment.
+ To work within the policies of the Trust.
+ Maintain comprehensive records of tissue viability related interventions
+ Assist the Tissue Viability Senior Nurse in maintaining service delivery audit.
+ Ensures dissemination of evidence based tissue viability related practice to all disciplines, as agreed by the Tissue Viability Senior Nurse.
+ Participates in research projects where applicable to improve practice.
+ Critically evaluate and analyse research and audit findings across a range of topics/options and implement best practice.
+ Formulate a thorough treatment plan, within agreed parameters and in line with Trust guidelines.
+ Light physical effort/ movement during wound assessment.
+ Movement of educational equipment to various location sites
+ Movement of Tissue Viability patient information between departments.
+ Concentration required on clinical assessment or delivery of education.
+ Support staff in clinical areas in delivering complex wound care.
+ Participate in clinical practice development projects
+ Support patients undergoing complex wound related procedures Apply specialist knowledge to patient assessment and interpret complex medical conditions in relation to Tissue viability.
+ The post holder may be in contact with body fluids.
+ The post holder will have limited exposure to a VDU
