This is a fantastic opportunity to become a key member of the Tobacco and Alcohol Care Team as an adviser, where you'll play a vital role in delivering assessments and providing targeted, impactful advice to patients across LTHTR. In this position, you'll collaborate closely with a dedicated team of advisers and specialist nurses, ensuring that patients who smoke receive the highest standard of care.
We are looking for someone with the passion and drive to work with a complex and often vulnerable patient group, striving to achieve the best possible outcomes. Your role will be essential in enhancing the quality of care, while also contributing to the broader mission of tackling preventable illnesses, reducing health inequalities, and easing the demand on healthcare services.
You will be required to submit evidence of your qualifications for this position. If your qualifications were obtained overseas, please ensure that you acquire a Statement of Comparability from UK ENIC to facilitate a clear understanding of how your qualifications align with UK standards. Your role involves providing specialised advice on specific conditions and treatment pathways, enabling the delivery of tailored care that suits each patient's unique situation. A key focus will be on continuously improving patient care and contributing to the development of high-quality healthcare services.
In this role, you will assess patients holistically, planning, implementing, and evaluating evidence-based care plans that address all aspects of their well-being. Additionally, you will offer valuable advice and support to patients, their families, carers, and the wider multidisciplinary team, ensuring everyone involved is informed and empowered.
You will also play a crucial role in workforce development by delivering education, training, and expert advice, thereby helping to build a skilled and knowledgeable team. Your commitment to providing excellent care with compassion will ensure that every patient feels understood and valued. Moreover, you will work collaboratively with the multidisciplinary team (MDT) to ensure that patient management is cohesive, coordinated, and effective.

+ Conduct assessments and develop personalised care plans for individuals seeking to reduce harm from tobacco, or for those needing temporary abstinence during their hospital stay.
+ Actively support a culture shift towards a smoke-free environment, leading by example and promoting this vital change.
+ Provide advice and support to ward staff, reinforcing training given to inpatient teams on reducing harm from tobacco and promoting a smoke-free site.
+ Ensure robust discharge planning for patients wishing to continue treatment after discharge, linking them with Smokefree Lancashire or a community pharmacy option.

Disclosure and Barring Service checks
If the role you've applied for requires a Disclosure and Barring Services (DBS) check we will administer this on your behalf and the cost will be recovered from your salary, either as a one-off payment, or over 3 months. The level of check required depends on the role that you have been offered. Currently the charges are - Basic DBS check £21.50, Standard DBS check £21.50 and Enhanced DBS check £49.50.
