Full Job Description

We require a motivated and dedicated Registered Nurse to work as part of the Tobacco and Alcohol Care Team.
The Tobacco and Alcohol Care Team is an exciting service which has introduced the CURE programme at Lancashire Teaching hospital. The introduction of the CURE programme will provide a medicalised approach to treating tobacco dependency.
This involves working in a team across both sites to ensure our service is equitable, responding to referrals and delivering training for trust staff. The Tobacco and Alcohol Care Team will work collaboratively with other teams and you will be required to liaise with other health professionals both within the trust and with the community drug and alcohol recovery services, stop smoking services, other network smoking cessation teams and other community supports services if required.
You must have the motivation and desire to work with a complex and often vulnerable group of patients to achieve the best possible outcomes, improve the quality of care and contribute to the overarching agenda of tackling avoidable illness, reducing health inequalities and reducing demand on services.
This is a great opportunity to gain experience in a stimulating and supportive environment, where you can further develop and actively contribute to the delivery of a high standard of patient care., You will be expected to assess patients who are referred to the service and provide appropriate pharmacological and psychosocial interventions, working towards positive outcomes for the patient. The ability to deliver training and advice on all aspects of alcohol and tobacco related health problems, to all grades of staff, patients and carers, is an integral part of this role.
You will be expected to provide clinical expertise and knowledge relating to alcohol and tobacco dependency treatment and should have an interest in developing your skills in this area. You will be expected to work autonomously but also in collaboration with the existing team members, CGL Inspire Prevention and Recovery Service, Smokefree Lancashire and the wider MDT, helping to achieve the targets of improving the quality of care offered to patients when admitted to hospital with alcohol and smoking related problems, reducing admissions and length of stay.

The current Tobacco and Alcohol Care Team consists of a team of specialist nurses working seven days per week across both Royal Preston Hospital and Chorley District Hospital. We have had the fantastic opportunity to expand the service to deliver interventions for Tobacco and Alcohol addiction combined.

You'll make an impact, be challenged to think differently, be bold & help innovate to keep improving things., + Offer specialist holistic assessment and plan, implement and evaluate evidence based care.
+ Where appropriate ensure the risk of alcohol withdrawal syndrome is assessed, communicated and escalated where appropriate, with appropriate pharmacological interventions implemented.
+ Provide specialist tobacco and alcohol harm reduction interventions including smoking cessation support and very brief advice to patients, staff and in some instances relatives/carers of patients.
+ Proactively support culture change and the promotion of a smoke free environment.
+ Co-develop suitable and effective training materials for staff and develop a training framework and delivery plan to ensure effectiveness of the service
+ Organise and deliver training for ward based staff through different forums including drop in sessions, ward visits, and formal training sessions.
+ Contribute to the work with the Trust's Communications and Engagement team to develop and promote internal and external communications around the service.
+ Ensure appropriate data collection to support clinical care and monitoring of the service.
+ Support the development and delivery of the Trust's Treating Tobacco action plan.
+ Support delivery of the Trust's alcohol-use disorders guideline.
+ Contribute to the evaluation of the service and ensure continuous quality development.
+ To have a designated case load and coordinate and plan the care for that group.
