Full Job Description

As aSeniorClinicalNurseSpecialistyou willwork within the Trust's Tobacco and Alcohol Care Teamto provide aseven dayservice for admitted patients whorequireinterventions and treatment for alcohol and/or tobacco use.

The Tobacco and Alcohol Care Team is an excitingservice whichhasintroducedthe CURE programme at Lancashire Teaching hospital along withthedevelopment of the existing AlcoholLiaisonService. The introduction of the CURE programmehasprovideda medicalised approach to treating tobacco dependency and providedan on-site service forpatients.

This will involve working inand leading theteam across both sites to ensure our service isequitable,responding to referrals and deliveringtraining for trust staff. The Tobacco and Alcohol Care Teamwillwork collaboratively with otherteamsand you willbe requiredto liaise with other health professionals both within the trust and with the community drug and alcohol recovery services, stop smoking services, other network smoking cessation teams and other community supports services ifrequired.

As part of the senior managementteamyou will contribute to the provision and development of theTobacco and Alcohol Care Teamby providing leadership and contributing to theoperational management of the Clinical Nurse Specialists andthe Tobacco and Alcohol Treatment advisors. You will support the team to care coordinate an identified caseload of both lower threshold and complex patients experiencing alcohol related problems and/or tobacco addiction.

You must have the motivation and desire to work with a complex and often vulnerable group of patients to achieve the best possible outcomes, improve the quality of care andmake a considerable contributionto the overarching agenda of tackling avoidable illness, reducing healthinequalitiesand reducing demand on services.

Responsibilities include

  • Provision of leadership, supervision and support to the Tobacco and Alcohol Care Team members

  • Offer specialist holistic assessment andplan,implementand evaluateevidence basedcare.


  • Whereappropriate ensurethe risk of alcohol withdrawal syndrome is assessed,communicatedand escalated whereappropriate,withappropriate pharmacologicalinterventions implemented.


  • Provide specialist tobaccoand alcoholharm reduction interventions including smoking cessation support andvery brief advice to patients, staffand in some instances relatives/carers of patients.

    Duties include

  • Take responsibility to ensure the team provide timelyresponses to referrals through triage andidentifyingtheappropriate pathwayfor intervention.


  • Contribute to, promote,educateandmaintainprofessional and clinical standards


  • To have a designated case load and coordinate and plan the care for that group.


  • Toprovideexpert advice related to specific conditions or treatment pathways.


  • Focus on improving patient care and service development


  • To assess patients holistically, plan implement and evaluateevidence-basedcare.


  • Toprovidespecialist advice to patient,families and carers and the wider multidisciplinary team


  • To contribute to the development of the workforce through developing and delivering education and training.


  • Toparticipatein research activity and clinical audits in own specialist area.


  • Deliver excellent care with compassion.


  • To work collaboratively with the multidisciplinary team (MDT) to ensure the cohesivemanagement of patients.


  • To act as a key worker for designated patients asappropriate acrossthe patient's pathway.


  • Deliver a range of clinical and core nursing skills and interventions to meet physiological andpsychosocial needs.

  • Provide supervision and support to the members of TACT

  • Deputise for the Tobacco and Alcohol Care Team Lead Specialist Nurse when needed.
