Full Job Description
This role is currently fixed term until 31st March 2025 with the intention that this period is used to evaluate and inform the transition to and adoption of a new Transfer of Care Hub model.
Those with current NHS service will be offered on a secondment basis only. Please obtain permission to be released on a secondment before applying.
This is an exciting opportunity to be able to contribute and shape the development of a Transfer of Care Hub within acute hospital sites across Dorset. You will provide a consistent presence within the acute hospitals for the Transfer of Care hub. You will work closely with the Cluster leads, acute hospital teams, social workers and community providers to ensure patients have a smooth transition onto the correct pathway contributing towards discharge and flow across the system.
You will be the consistent presence at one of three hospital sites on behalf of Dorset Healthcare. The three sites are: Dorset County Hospital in Dorchester, University Hospitals Dorset both Poole and Bournemouth sites.
You will be responsible for triaging referrals sent to single point of access (SPA) within Dorset Healthcare Trust and liaising with all teams involved in planning discharges and agreeing patient pathways.
Your role will involve liaising with patients and families on the ward when needed to expedite discharges.
This is an exciting opportunity for someone who is keen to build supportive and strong relationships across partners and in turn make a difference to patient pathways across Dorset.
