Full Job Description

This role will appeal to an experienced Nurse or an experienced Children's Social Worker, Social Care and Health Practitioner.

We are looking to recruit a dynamic and enthusiastic individual who can take the lead on the operational delivery of the Transition Service. You will work alongside the wider teams at Hope House and Tŷ Gobaith, and external care providers.

What we are looking for a Transition Lead Practitioner who:

To lead on the development of the Transition Service which promotes empowerment and independence for young people in planning and preparing for their future.
Work in Partnership with those who may have statutory and legal responsibility for the young people in order to ensure a successful transition between services.
Always maintain a professional attitude, developing the role in the best interest of the young people and profession.
Foster a culture of inclusivity respecting the diverse needs of young people and families and reducing inequalities of access to care and care delivered.

This role requires you to demonstrate effective leadership skills, you will need to be able to multitask and possess excellent communication and people skills. You will need to be flexible and adaptable in your approach to work and demonstrate effective decision-making skills throughout., Disability Confident

Hope House Children's Hospices have an opportunity for a Transition Lead Practitioner to join our team based in Oswestry. You will join us on either a full-time or part-time, permanent basis. In return, our Transition Lead Practitioner will receive a competitive rate of £37,088 - £44,085 FTE per annum (dependant on experience).
