Hammersmith & Fulham has developed the scope of its Transitions Team and, at this exciting time, it is seeking highly motivated qualified Social Workers to support the Council in driving through a new approach to working with young people as they transition through the Council's Children's Services into Adulthood.

As well as working within the Statutory and Local Guidelines, and having a strong understanding of Safeguarding, the role will require creative, community-based planning, working with young people and their families to effectively plan for adulthood, ensuring that they are supported to navigate the often complex route into Adult Services.

As a new team that will offer both planned and reactive support to this age group, we are seeking an experienced Senior Social Worker that can evidence significant and pro-active multi-agency experience. The team will collectively have overarching knowledge across the key areas that might affect planning and subsequently impact on outcomes for this client group, including Special Educational Needs and Disabilities in Children's and Adult services, Health (including CHC funding), Care Leavers, Housing and Direct Payments. We are, therefore, keen to recruit people with specialist knowledge and experience in any of these areas. At this level the role holder will be more autonomous. They will demonstrate expert and effective practice in complex situations, assessing and managing higher levels of risk, striking a balance between support and control, liaising with a wide range of professionals, including more senior levels.

Additionally, as the post will be joining a new team, it is key that the post-holder is willing and able to work flexibly whilst the new policies and procedures are embedded into business as usual.

-Recognised social work qualification (DipSW, CQSW, CSS or approved equivalent, BA Hons Social Work or other relevant degree in Social Work).
-Social Work England registration.
