As a recognised provider of innovative, high quality services Central London Community Healthcare NHS Trust are looking for exceptional individuals to join our highly respected Adult Community Nursing Service Team. The location of the post is St Charles Centre for Health, W10 6DZ and cannot be worked remotely. We have vacancies for triage nurses, in the Night District Nursing Team in Inner NorthWest London covering the boroughs of Hammersmith & Fulham, Kensington and Chelsea and Westminster. We wish to recruit Band 6 community staff nurses who are committed, forward thinking and enthusiastic to cover the triage service which runs from 18.00 - 21.00, 7 nights a week. We envisage this as a job share between 3 - 4 individuals. The successful candidates will be highly motivated and compassionate, caring for patients and their families in often stressful situations. You will be skilled in triage and telephone assessment; be able to work on your own initiative, triaging and prioritising patients with the ability to recognise when to escalate emergency concerns.Main duties of the job To provide skilled triage and telephone assessment of patients referred into the Night District Nursing Service covering Westminster, Kensington and Chelsea and Hammersmith and Fulham, between 18.00 and 21:00. To work alone between 18 - 20.00 accepting referrals from multiple sources, registering on SystmOne, in paper diary and message book and contacting patients and relatives. To be a point of contact for CLCH late call/twilight services across the Tri borough and all services who refer into the Night District Nursing team. Using triage skills to recognise when to escalate urgent and emergency concerns. To be able to troubleshoot over the phone with patients, carers, clinicians e.g. with syringe drivers, urinary catheters, or medical equipment. Provide a comprehensive handover to staff at the start of the night shift ensuring essential information is recorded on SystmOne and communicated verbally to the night team. Have an excellent working knowledge of key stakeholder services such as District Nursing, London, Central and West Unscheduled Care Collaborative, Out of Hour GPs, local hospices, London Ambulance Service, My Care My Way, Community Independence Service, NRS equipment provider, Social Services, Safeguarding, alarm services etc within Inner NorthWest Division knowing how and when to refer into these services and processing incoming referrals. To monitor stock levels of equipment, order and receive deliveries and perform stock checks.
