Triage Specialist Nurse

We are looking for an experienced and versatile Triage Specialist Nurse with excellent communication and interpersonal skills, to join our Hospice at Home team. You will work alongside the Triage Lead managing referrals efficiently, enabling the Princess Alice Hospice at Home team to be responsive to patients and carers in a timely way., You will be comfortable acting as a specialist advisor for other healthcare professionals, patients, and carers. Youll enjoy liaising directly with different referrers (including the Primary Care team) so you can ensure patient needs are holistically met.You will feel confident in holding a telephone caseload of non-complex patients to help support the Clinical Nurse Specialist team in caseload management.

You will have a relevant degree, diploma or be in the process of studying for one. Palliative care or oncology experience is essential to ensure a solid understanding and relevant clinical knowledge for you to draw upon.
