Full Job Description

In line with our clinical strategy we have developed an Elderly Assessment Unit at Wansbeck Hospital to which supports the move to alternative pathways for urgent frailty assessment and redirects patients to the most appropriate environment for their assessment and treatment resulting in more people remaining within their home environments. This is achieved through the following:

  • Same day direct access pathway for NEAS and Community services including Primary care
  • Virtual ward - step up facility to assess patients of concern in the community, step down facility to enable positive risk taking for patients in the emergency department and NSECH wards
  • 2 hour response - step up facility to assess patients of concern to community teams
  • Single point of contact for urgent advice for frail patients with aim of utilising urgent assessment services and community teams rather than NSECH emergency department if appropriate
  • Referrals from GP/Community Services within 2 week wait providing a semi urgent response., We recognise the positive value of diversity and inclusion and are committed to a workforce that is diverse, equal and inclusive. We welcome and encourage job applications from people of all backgrounds. We particularly welcome applications from Black, Asian and Minority Ethnic (BAME) candidates as BAME people are currently under-represented in our workforce as well as other under-represented groups such as LGBT+ and disabled candidates.
