Full Job Description

The post involves providing high quality care to adults, children and babies. You will work both autonomously and part of the team adopting a multi-disciplinary team approach, maintaining close links with ID services, Children's services, microbiology, other TB teams, community agencies and the UK Health Security Agency. You will run screening clinics within both the hospital and community setting.
The successful candidate will be required to coordinate contact screening by identifying close contacts of index cases and facilitate screening thereafter. You will undertake nurse led clinics, undertaking holistic assessments applying national and local guidelines; you will manage a caseload of patients receiving treatment for TB.
You will also be expected to review patients in their own homes undertake compliance and welfare checks and complete outreach work and participate in the Latent TB programme in the community. You will deliver the BCG vaccination to eligible infants, children and adults .You will be a resource to others; providing training and support to other health care professionals. You must be eager to undertake working in new ways packages, venepuncture and requesting chest x-rays. It is essential that you have your own car and full drivers licence., + Assess the care needs of a specialised group (TB) of patients and develop a programme of care, which is evaluated and amended as appropriate.
+ Delivers BCG vaccinations to eligible individuals and provides education and support to parents, carers and other healthcare professionals.
+ Actively support equality and value diversity. Utilise interpreting services appropriately to enable effective communication with patients from ethnic minority groups where English is not their first language.
+ Carry out relevant forms of direct and indirect patient care autonomously working at advanced level practice.
+ Takes charge of a caseload of patients providing enhanced case management independently whilst supporting other colleague's workload in their absence.
+ Actively contribute to setting and maintaining high standards of quality nursing care, and maintenance of local and national TB databases, including analyses of results.
+ Undertake any "Working in new ways packages" such as venepuncture, x-ray package as directed by the TB lead.
+ Actively participates in the education, development and supervision of other staff members
+ Act as a clinical and professional role, assessing and supporting students and learners on placement within the clinical area.
+ Work collaboratively and co-operatively with others to meet the needs of patients and their families.
+ Deputises in the absence of the TB lead at strategic or National level.
+ Utilises and integrates evidence into practice.
+ The post holder is required to be registered with the Nursing and Midwifery Council.

This post is an ideal opportunity for a motivated individual to join our experienced forward thinking team. Gaining expertise in all aspects of TB (Tuberculosis) disease is fundamental for driving the service forward and meeting the local needs of the community. Screening and the prevention of TB are key elements of the post and you will be required to work autonomously in the clinical setting and community with support from the existing TB team. Part of our work is also to administer the BCG vaccination and therefor a desire to build on existing clinical skills in vaccination and new ways in working (venepuncture/ ordering of chest x-rays) is essential. As always communication is central to any service, however we rely on effective communication and collaboration with many other key stakeholders and divisions within the hospital and community to support the patient's journey. If you are aged 16 or 17 and applying for a role that is more than 20 hours a week, please be aware that you will be asked to commence an Apprenticeship within the Trust alongside your role, as long as there is a suitable apprenticeship standard available. Salary: The quoted salary will be on a pro rata basis for part time workers.

+ Disclosure and Barring Service: Your post maybe subject to a DBS check which incur a cost dependent on the level of check required (£42.90 for enhanced and £22.90 for standard).
