Full Job Description

The Sirona TB Nurse Team work collaboratively with patients and a wide range of colleagues to provide high quality care for people affected by TB across the Bristol, North Somerset and South Gloucestershire localities. We are proud of our high treatment completion rates and the feedback we receive from patients.,

  • Takes lead responsibility for the case-management of TB cases, assessing need and implementing and evaluating individual programmes of care and support for people being treated for TB
  • Work in community clinics, patients' own homes
  • Maintaining close liaison with GPs and hospital services
  • Responsible for collating accurate data and meeting performance targets, contributing to research and audit
  • Recognise patient perspectives, seek their views and feed those into service development
  • Provide education programmes and presentations on TB and related issues within Sirona and the wider health/social community as required
  • Attend TB clinics in the acute trusts alongside own patient list
  • Communicate effectively with GPs, patients and use IT systems effectively to enable care
