Full Job Description

The Crawley Twilight Team are based in Crawley Hospital and are part of the Overnight Community Services covering West Sussex . The Crawley Twilight team provide nursing care to patient's in their own homes within West Sussex. You would work alongside a registered nurse, providing urgent interventions to patients requiring symptom management for end of life care, urgent catheterisation, admission avoidance for deteriorating patients, urgent wound care and medication administration. Our shifts commence at 18:00hrs and finish at 23:00hrs which works well for parents of school age children. We welcome a call or visit from interested applicants so that we can answer any questions you have. We also offer great development opportunities for our Health Care Assistants and we are passionate about supporting you throughout your career. Assisting the registered nurse with nursing interventions. You will be required to learn skills such as administering some injectable medications, providing wound care, advanced communication skills and IT skills to support you with documentation in patient records and E-learning. We also require you to be a car driver and flexible within your shift as to where you will be required to attend to a patient. Working collaboratively with the Multi-disciplinary Team to support patients towards rehabilitation, self-management and independence throughout the continuum of care. b) To work as part of a the Twilight Team to provide health care and support under the direction and supervision of Registered Nursing staff to patients living in the community. This includes assisting in the ongoing assessment and provision of care and reporting all changes in patient status to senior staff. c) To provide administrative support to the Twilight Team as appropriate.
