Full Job Description

The post holder is responsible for the continuing management of the twilight patients and/or co-owners within their clinical setting, without supervision. This includes the assessment of careneeds, the development, implementation and evaluation of programs of care, the setting andmaintaining of standards and the supervision and deployment of the twilight team, if appropriate.The post holder will undertake the facilitation of educational programs for basic, post-basic and in-service training, in close liaison with the Learning and Development Team. To work closely with the Community Hub teams to facilitate patient-focused, innovative research-based practice, inline with Surrey Downs Health & Care objectives. To promote continuity of care between twilightand day services through effective communication and processes. To coordinate closely with allSurrey Downs Health & Care twilight teams to ensure a joined-up (seamless) service is deliveredand the teams are supported., Patient Care and Assessment:

  • Provide skilled nursing care, assessing needs, and creating, implementing, and reviewing care plans.
  • Treat patients with respect, ensuring privacy and dignity.
  • Promote patient health and well-being through education.
  • Team Leadership and Collaboration:
  • Plan staffing and ensure adequate cover.
  • Coordinate meetings and duty schedules.
  • Ensure continuity of care through effective communication.
  • Mentor and support staff.
  • Documentation and Compliance:
  • Maintain accurate records.
  • Adhere to policies and standards.
  • Support clinical supervision.
  • Research and Development:
  • Engage in nursing research.
  • Plan and evaluate staff training programs.
  • Operational and Resource Management:
  • Manage budget and resources.
  • Participate in recruitment.
  • Ensure adherence to clinical governance and risk management strategies.
  • Professional Standards:
  • Maintain professional integrity.
  • Facilitate a multi-disciplinary approach.
  • Undertake additional duties as needed.
