Full Job Description

We are delighted to be recruiting to our substantive Urgent Community Response (UCR) Therapy Team, where you will be one of at least four B7 Therapists, supported by non-registered members of the team. With a passion for urgent patient care in the community, this is the perfect role for anyone excited by complex clinical situations, with varied and at times unpredictable workload. Having been running locally for 6 months, we are now recruiting to permanent positions where you will be helping to shape the future of the service and its delivery, as we plan to expand. Under the umbrella of Intermediate Care, UCR will be encouraged and facilitated to adjust and develop to the needs of the local population. You will receive clinical supervision within UCR, working in conjunction with Hospital at Home, and have regular 1:1s with the Service Manager. We will support your ongoing CPD through both formal relevant education (Masters) courses and informal opportunities across the ICS. The service operates between 8am-8pm 7 days a week with a mix of Long Day (8am-8pm), Early (8am-4pm) and Late (12pm-8pm) shifts. A current UK driving license is preferred but not essential. Candidates with a driving license will require business insurance and will be asked to drive the Trust van and/or pool cars. We will happily accommodate visits to the team, any time within current working hours. Please note that we are unable to provide visa sponsorship for this position. Clinical 1. Complete specialist, complex assessments demonstrating specialist knowledge of the concepts of biological, physical, social, and psychological sciences in relation to function to ensuring patient safety in their own homes 2. Complete comprehensive holistic assessments to address immediate presenting needs of the patient utilising extended clinical skills 3. Complex manual handling including urgent prescription of specialist equipment to manage immediate risks to patients during a crisis situation 4. Profession-specific assessments, generalist assessments and, where appropriate and within scope of extended practice, cross-profession assessments to maintain patient safety during acute medical / social crisis 5. Refer to appropriate clinical services to ensure patient safety is maintained; escalation of patient deterioration 6. Use assessment tools/skills that will ensure an appropriate level of recovery intervention so that patients who present with highly complex needs are timely referred to the appropriate clinicians/teams 7. Complete complex risk assessments, both as an independent clinician and drawing on the wider MDT as appropriate 8. Triage and prioritisation of new referrals and allocation of workload 9. To be responsible for the day-to-day management and prioritisation of your own caseload 10. Respond to urgent referrals and an unpredictable workload: this will require prolonged concentration in an unpredictable environment 1. Manage potentially stressful, upsetting or emotional situations in an empathetic manner including when imparting bad news: to ensure this is communicated sensitively to all parties concerned 2. To keep yourself well at work both physically and emotionally, to access peer, clinical and management support when needed and to be aware of own limitations and escalate accordingly 3. To work as part of the wider Intermediate Care team in community services as required 4. Be wholly accountable for your clinical practice taking every reasonable opportunity to sustain and improve knowledge and professional competence 5. To manage a varied caseload, seeking support from peers and senior staff where necessary to ensure the provision of a timely, clinically effective service based on evidence based practice 6. To be actively involved in service development projects towards the development and delivery of UCR, therapy and broader community services, and to contribute to new developments and initiatives within the locality 7. Play an active role in the management of clinical governance and patient safety Management 1. To be part of the Duty Manager rota within Intermediate Care being responsible for the day to day needs and management of the wider service 2. Line Management and supervision of junior staff (registered therapists and/or non-registered staff) and students, including performance management, objective setting and appraisals 3. Participate in the implementation of departmental policies and the annual performance and service development plan 4. Be an active participant in the Trust's appraisal scheme 5. Undertake investigations of complaints where required, participate in the risk management process, critical incident reporting, evaluation, action planning, dissemination and change in practice. 6. Take responsibility for cost effective management and safe use of expensive and highly complex equipment, provide recommendations for effective use of resources and contribute to the effective delivery of cost improvement planning. 7. Maintain and provide accurate and timely statistics to support service development and provision 8. Deputise as clinical lead for Intermediate Care if Team Leads are absent 9.
