Full Job Description

The post is based within the Rapid Response Team. This service provides a multi-disciplinary urgent assessment and crisis intervention service within a 2 hour response, and at times a 2-6 hours response to patients in the community with the aim of preventing hospital admission operating 7 days per week 8am to 8pm. Our service is looking for a proactive and enthusiastic staff nurse who is excited to be part of our team and who is willing/interested to progress their career. The post holder is a Nurse should have good community experience and is skilled to see patients independently, making autonomous decisions in the clinical and diagnostic assessment of patients with potentially complex health and social care needs to support keeping them in the community and reduce hospital admissions / ED attendance and to develop a comprehensive plan of care. The post holder will have the support of the Advanced Practitioner/GP where there is uncertainty or a need for additional support. The Referrals are accepted to service from GPs London Ambulance Service (LAS), care homes, and community providers.,

  • To comply with the duties placed on employees by the Bromley Healthcare's Health and Safety Policy and Related Procedures and to act in accordance with all instruction, information and training supplied in relation to those duties.
  • Such other duties as requested, compatible with the post, which may arise from time to time and flexibility to support the services within UCR.
  • Observe the confidentiality of records and health information at all times and be aware of and practice the principles of the Data Protection Act of 1984, as this applies to computerised information.
  • To undertake appropriate training to meet individual developmental needs within the role.
  • Undertake the measurement and evaluation of work and current practices as directed by team lead as part of the clinical audit programme.
  • To work with the Rapid Response Lead /ANP /ACP to facilitate changes in practice across the service to improve clinical outcomes to meet the needs of patients and carers.
  • To respond to urgent referrals into the service from GP/111/LAS/Care Homes and HCPs for patients to be seen at home, who have deteriorated below baseline due to illness or injury and will require additional clinical support, medication prescribed and care plans to prevent attendance to ED or hospital admission.
  • To participate in achievement of service objectives to demonstrate productivity, performance and effectiveness
