Full Job Description

An exciting opportunity has arisen within our Urgent Community Response/Virtual ward Service for a Senior Nurse, 32 hrs per week, 12 month fixed term contract.

Our UCR/VW is an evolving service delivering acute assessment, management and monitoring within the community setting and we are looking for a dynamic forward thinking individual to join the team.

The post holder will work collaboratively within the multi disciplinary team to offer holistic high level clinical assessments and reviews for patients who may be in a crisis situation., Urgent Community Response is a service operating a multidisciplinary approach, supporting patients in their own home for 72 hrs and so preventing ED attendance and unnecessary hospital admissions. The team is made up of Senior Therapists, Senior Nurses, Social Workers, Nurse Associate, Community Pharmacist and Senior Support Workers.

o The role of the Senior Nurse is to work collaboratively with other team members offering holistic high level clinical assessments and reviews for patients who may be in a crisis situation. There is a strong element of "See and Treat" management within the nursing role in the community setting. of care to prevent hospital admission

o Demonstrate a high level of judgement, discretion and decision making within the assessment process with an aim to reduce hospital attendance/admission

o Develop and deliver educational/training programmes and act as a specialist resource for staff and colleagues to improve standards and quality of care.

o Monitor and improve standards of care through supervision of practice, clinical audit, evidence based practice, learning and development and the provision of skilled clinical leadership

o Support the service lead with service improvement development
