Full Job Description

You will work as part of an integrated health and social care team providing a crisis intervention and early supported discharge service for patients across the City of Southampton registered with a Southampton City GP practice

To support the Clinical Lead, Nurse Team Leads, Urgent Response Occupational Therapist and Physiotherapist in providing clinical knowledge and leadership to the Urgent Response Service, supervising, and developing junior staff and contributing to the achievement of divisional business delivery objectives

To working collaboratively with partners in health and social care, to establish a whole systems approach to stop a health and social care crisis or lead in effective Community Pull model effecting early and safe discharge from hospital following an acute episode of care, To support the Early Supported Discharge from hospital by assessing patients in their own home and supporting with intense therapy as required

To be responsible day to day for a designated case load of clients requiring urgent intervention and organise and prioritise the required programmes of treatment

Work within the community and in UHS ED as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility, knowledge, or skills

Undertake multi-factorial assessments using the Single Assessment Process and undertake specific Physiotherapy assessments for clients by using investigative and analytical skills for those with acute/complex/rehabilitation or palliative care needs e.g. clients with multiple pathology and complex social care needs

Formulate individualised management and treatment plans using sound knowledge and evidence-based practice. Plans should being response to sound clinical reasoning and a range of treatment skills and options to formulate specialised programmes of care

Assess Safeguarding risks and refer through the appropriate route both in and out of hours using recognised pathways and policies Attend Safeguarding meetings as appropriate to ensure effective co-working with other community teams
