Full Job Description

  • Please note that this job involves equal time working face to face with acutely unwell adults across the county and online working
  • Responsible for the assessment, planning, implementation, and evaluation of care without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life. To work in partnership with patients and carers to offer support and advice and promote informed choice. To support the role of the ward manager and band 6 staff. To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures, and guidelines.     We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health.
