An exciting opportunity has arisen to work within the Adult mental health community services at a time of innovation and service development. This role will be involved, as part of the wider core community teams, in helping to adapt the existing service within the new transformation process. This process is in response to the NHS 5-year plan to develop mental health services round the Primary Care Networks and provides new opportunities which include developing partnership working with the voluntary sector, working more closely with GP practices and developing more innovative, community based and holistic ways of working.

The post holder will be responsible for participating and assisting in the delivery of care/services to patients, service users and carers as part of the Core Community Mental Health Team. The post holder will also be part of a daily Core Mental Health MDT referral meeting where all new referrals will be screened and allocated to the relevant Mental health practitioner/named worker, for a mini- Dialog+ assessment or full Dialog+ assessment and care plan - depending on presenting needs You will be responsible for carrying out practical tasks, identified as part of the service users' care plan or care pathways and delivering interventions as part of an initial offer to our service users. As a Wellbeing Practitioner you will be assigned to a PCN and will work closely with the PCN Mental Health Practitioner on advice/support for referrals.

  • To Be part of a daily Core Mental Health MDT referral meeting where all new referrals will be screened and allocated to the relevant Mental health practitioner/named worker.

  • To undertake Mini DIALOG + and Full DIALOG +

  • Carry clinical caseload of service users (of approximately 30 cases at any one time) for up to 6 weeks, seeing initial assessments within 4 weeks for DIALOG + or DIALOG+ care plan in line with the national performance indicator (KPI).

  • To ensure that people referred for intervention are quickly and efficiently able to access the most appropriate treatment for their mental health difficulty

  • Participate in co-running psychosocial and community focused groups as part of a rota and with oversight from psychology, complex care pathways and with the community engagement workers.

  • Deliver 1:1 support/interventions based on an individual Dialog+ care plan, with short term outcomes for service users

  • To work closely with the PCN Mental Health Practitioner on advice/support for

  • To Work closely with the core teams to enable both the prevention of further access to Mental Health services as well as participate in the delivery of the discharge framework, supporting move on from service - supporting the continuity of care for a person recovery journey through services.

  • To work with other clinicians, in providing effective, evidence based therapeutic care, to lead or support other clinicians with the development of care pathways.

  • To help service users manage their symptoms, deliver evidence-based interventions and give advice and support on issues such as housing, benefits, education, employment, medication and social activities in the local area.

  • To be proactive in ensuring service users receive the range of interventions they need in line with their care plan

  • To participate fully in caseload supervision and management, to ensure that care plans are matched to need and service users are able to 'flow' through pathways of care.

  • To be flexible and adaptable in order to respond to the diverse and demanding nature of this


  • Communication & Information Management

  • Maintain clear, comprehensive and contemporaneous clinical records on all patients in response to personally generated clinical observations, in accordance with professional standards and Trust policies.

  • To liaise with other agencies in planning and assessing care needs associated with specified treatment pathways.

  • To ensure that all communication takes place in a manner that is consistent with legislation, policies and procedures.

  • To provide verbal and written communication with the multi-disciplinary team to co- ordinate effective service user care or service/clinical development, including referrals and sharing of care plans where appropriate to support integrated, patient-centered care, in line with Trust


  • Responsibility for ensuring that appropriate methods are initiated to facilitate effective communication with patients and informal carers who may experience communication difficulties g. those who have mental health issues, learning difficulties, cognitive or sensory impairment or those who require interpreters.

  • People (HR) Management

  • Provide formal, ongoing clinical supervision and caseload management supervision to junior and less experienced community mental health staff and students, as directed.

  • To take responsibility for receiving regular supervision in accordance with Trust

  • To maintain a safe environment, escalating concerns and participating in health and safety reviews as required.
