An opportunity has arisen for a Well Being Team Coordinator to contribute to our services within the Adults and Health directorate of the Council. Wellbeing Teams provide care and support for clients in their own homes, single and double handed calls. The service provides care in the community between the hours of 7am and 11pm.
You will be required to work across the Tilbury and Chadwell Teams providing support by line managing the Well Being Workers. You will have experience of rostering care calls to staff, and of working with multi-disciplinary teams of Care and Health Professionals. We are looking for a committed and innovative individual with the knowledge of how a Well Being service operates. You will have worked in statutory services and have a solid knowledge of safeguarding and CQC requirements. In addition, you will have experience of leadership (you will be responsible for a team of at least 15 staff).

The role will be working to promote personalisation and re-ablement where possible, whilst assisting in achieving the delivery of outcomes for residents within their own home. You will need to be able to build appropriate relationships with Service user's and staff to ensure core values are met.
The type of person who will be suitable for this role will need patience, understanding, commitment and have a genuine caring nature. You will also be able to demonstrate the core values of the home promoting Dignity, Choice & Control, Person Centred Care, Rights, Fulfilment, Respect and Independence. You will need to have a Full Driving Licence and access to your own vehicle to enable you to travel around the borough. A flexible approach to working hours will also be required to support the service outside of office hours.
Full Driving Licence and access to your own vehicle required for working in the above - mileage paid at 45p per mile.
The Council supports the Covid-19 vaccination programme and encourages staff delivering front-line care services to take up the offer of double vaccination.
The successful candidate will need to have a professional attitude at all times and have knowledge around the understanding of Dementia, Challenging behaviours and Manual Handling - training also provided.
A Disclosure and Barring Service (DBS) check for Adults is required at Enhanced plus Adults Barred List level
