Full Job Description

This is an exciting opportunity to work in a brand new role within The Vale of Evesham PCN. The role will give you a unique chance to work as a member of The Vale of Evesham PCN team, whilst also building links with a range of other local services and be part of a county wide Work and Health coach network. We will particularly welcome applicants with lived experience and those with strong connections to The Vale of Evesham area. The successful candidate will be part of the Workwell programme and will work with people from a range of backgrounds to help them return to work. They will work closely with participants within the local community to assess the barriers they experience in returning to work and provide support and coaching along with health and employment advice to overcome these barriers and return to work.,

  • The core purpose of the role is to support participants to Start, Stay and Succeed in work. The post holder will be expected to work flexibly, using their judgement, training and professional experience to achieve this goal. This may include the following non-exhaustive list of activities:
  • Receive and process referrals directly from Primary Care colleagues with the PCN the post holder is based in
  • Receive referrals from Job Centre Plus and Employers
  • Validate participants eligibility prior to offering support.
  • Manage a caseload of participants who have common mental, physical and social health problems or long-term conditions.
  • Carry out assessments to identify barriers and develop action plans in agreement with the individual to overcome those barriers
  • Provide coaching and other light-touch holistic support on a 1:1 basis to support the delivery of the participants plan,
  • Onward referral to other services where additional support beyond the 1:1 support is required. This may include a wide range of health and employment related support as needed to address the participants needs.

    This role would suit someone with previous experience of coaching and mentoring, clinical experience is not required and training will be provided for the successful candidate.

    The Vale of Evesham Primary Care Network first became established in April 2019 and consists of seven local practices working together to provide the best possible care to their patients. The Vale of Evesham PCN covers a population of approximately 62,000 patients, geographically covering Evesham, Broadway, Bredon and Inkberrow. The Vale of Evesham PCN is committed to the development and delivery of coordinated healthcare, working with partner organisations to support patients in making informed decisions about their personal care promoting health and wellbeing and ensuring equality of access for all.
  • It means providing innovative services that are designed around the needs of our community. We work collaboratively with local organisations in the healthcare and voluntary sectors, as well as in partnership across GP practices. This helps us to ensure were providing a comprehensive, joined-up service. SW Healthcare is a fast-growing provider of community-based healthcare services to NHS patients within South Worcestershire. The organisation utilises the excellent clinical expertise that already resides within the local NHS environment and harnesses it with efficient administration pathways to provide patients with more convenient and accessible services. We hope that by doing this we will not only deliver pathway improvements to patients but also go some way to relieving the growing volume pressures that are being experienced within local hospitals and GP practices.
