If you answered yes, we've got the perfect role for you This is a unique opportunity to work in our dedicated wound clinics which cover all types of wound and leg ulcer care across Medway. This post is a varied role and will involve delivering based wound care; full assessments, implementation and evaluation of wound and leg ulcer care. You will need to be flexible as the wound clinics operate 7 days a week based at Lordswood HLC. To work as a member of the tissue viability and wound therapy service to undertake the nursing care of patients attending Medway Community Healthcare wound clinics, Community nursing and Leg ulcer clinics To provide effective evidence based wound care including the assessment of care needs, the development, implementation and evaluation of plans of care, prescription of treatment and advice to patients who are seen by the service. To support wound therapist in the management of the clinics and take responsibility for the care and treatment of your patients. To oversee the healthcare assistants in the team as required and support the nurse therapist with allocated students and other visitors to the clinic. So what else?

· Registered Nurse on Part 1 of the Nursing Midwifery Council register · Evidence of further study · Mentorship or training qualification · Evidence of completion of tissue viability and leg ulcer degree module or equivalent or willingness to undertake at earliest opportunity · Recognised degree in nursing or equivalent qualification · Nurse prescriber or willingness to undertake training · Relevant post-graduate qualification, e.g. in elderly care, chronic disease management, long term conditions Experience · Post graduate accumulative experience in the fields of tissue viability or leg ulcer management /elderly/ community/ long term conditions. · Experience of delivering education and teaching. · Proven and evidence of experience in an autonomous practitioner role · Experience and competence in high-level clinical assessment of complex patients · Evidence of specialist knowledge in community nursing and advanced practitioner · Experience of using technology to facilitate provision of patient care · Experience of motivational interviewing · Change management experience Special knowledge / expertise · Knowledge of current issues and trends within community healthcare, tissue viability/leg ulcer management, long term conditions, intermediate care and the NHS in general · A working knowledge of the admission avoidance ethos and facilitation of timely discharge · A sound knowledge of clinical governance and risk management · Ability to practice at an advanced level · Experience of lone working anddecision making · Experience of mentoring students and other healthcare professionals · Proven ability to manage a caseload · Intermediate IT skills (Windows, Internet, Outlook, Excel ) · Presentation skills and teaching or experience · Research awareness and audit skills Experience of using research and other evidence to inform practice Disposition, adjustment, attitude and commitment · Self-motivated · Able to work and make decisions autonomously and take the initiative · Assertive but diplomatic towards colleagues, patients, carers and relatives with interpersonal skills which will encourage self-empowerment · Flexible and responsive to the needs of the service · Ability to recognise own limitations and to refer appropriately when necessary in accordance with professional code of conduct Practical / intellectual skills · Excellent verbal and written communication skills · Effective time management, organizational skills and ability to meet deadlines · Able to travel to dispersed sites in a timely manner MCH values Being caring and compassionate, Disability Confident About Disability Confident A Disability Confident employer will generally offer an interview to any applicant that declares they have a disability and meets the minimum criteria for the job as defined by the employer.
