Are you passionate about supporting the emotional well-being and mental health of children and young people?
Do you want to make a real difference as part of a dedicated team working within Exeter Primary Care Network?
At Young Devon, we are committed to improving the lives of children and young people. This is an exciting opportunity to join our wider well-being team, using your youth work skills to provide vital mental health and well-being support. You will be working on behalf of a group of GP practices within a Primary Care Network (PCN), helping to make a lasting impact on young people's lives.
You will be working for Young Devon, based in a PCN as a member of the integrated teams in GP surgeries. We are looking for competent practitioners, able to work autonomously and creatively in a busy and demanding role. You will help to reduce isolation and improve health and wellbeing through aspects of social prescribing, health coaching, care co-ordination and low-level psychological intervention. Supporting individuals by engaging in fun and developmental activities, helping them to access a range of support services and interventions to bridge the gap between Primary Care and Secondary Mental Health Services.
If you would like a chance to really make a difference for young people and have the skills outlined in the person specification, can engage them and help them to feel supported, stay connected and build their resilience, character and confidence then we want to hear from you.
