Full Job Description
- Lead and deliver the Healthy Child Programme within a designated population area
- assess health needs and implementing interventions with a focus on personalised or targeted responses, utilizing team skills effectively.
- Take accountability for children's outcomes collaborating with partners to address public health needs
- Promote health and well-being through the Healthy Child Programme and referrals to partner agencies as needed
- Contribute to multi-agency early support assessments for children and families, taking on a lead professional role when necessary.
- Manage a caseload autonomously ensuring effective assessment, intervention, and management.
- Model excellence in Specialist Community Public Health Nursing practice, providing supervision to learners and colleagues.
- Provide management and leadership to community nursery nurses and other team members, including caseload allocation and monitoring.
SCPHN qualification with current NMC registratio - Leadership experience in health visiting teams and implementing interventions for child and family welfare.
- Proficiency in health visiting tools/programs to enhance child outcomes.
- Valid driver's license and access to a car for work. You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
- £37,338 - £44,962 Band 6 salary with NHS Pension and full Agenda for Change terms and conditions
