Full Job Description

We are looking for a Mental Health Community Support Worker to work within the Specialist Teams for Early Psychosis (STEP) in Torbay and South West Devon.

The primary purpose of this role will find you helping improve the quality of individuals lives by supporting the individual (re)engage in the social networks and structures that are often lost during the early stages of psychosis.

Early Intervention services offer evidenced based intervention that reduce risk of illness. It is a bio-psycho-social model of care. The STEP service is a specialist multi-disciplinary team that work across both adult and young person's services within Devon Partnership NHS Trust. It includes Mental Health Nurses, OTs, Social Workers, Advanced Practitioners / Non-Medical Prescribers, Clinical Psychologists, Cognitive Behavioural Therapists, Support Workers and Peer Workers. We are committed to the Triangle of Care and many staff are trained in Family Interventions., The post holder will assist in enabling patients to access the support they need to address their social, physical and mental health needs. Promoting recovery, independence and social inclusion. The post holder will be working closely with teams in both Primary and Secondary care to enhance the existing provision and promote engagement. The post holder will work alongside the other members of the team, working in isolation as well as assisting in the provision of groups and family interventions.

The post holder will also be called on to complete physical health monitoring. This includes measuring blood pressure, height and weight, and being able to use a Point of Care blood test machine.
