Active Recovery Triage Coordinator

The SkILs Reablement service is a short-term service responsible for supporting people in their homes, to rebuild their confidence and independence and making sure they can live safely.

The team helps people to regain their independence where possible, to do everyday activities or with using equipment to help maintain their independence.

This is an exciting opportunity to join a busy and developing service.

We offer a comprehensive Induction Programme, regular one-to-one meetings, a strong commitment to your personal development and a strong and supportive team with effective leadership., To screen and manage all the referrals sent to SkILs Reablement service whilst working in the Active Recovery Triage HUB. To liaise with the Neighbourhood Teams to identify appropriate resources for customer visits and to coordinate the allocation of visits to the Case Officers. Undertake visits to the customers when necessary.
