Full Job Description

  • Annual salary of up to £53,219 (band 8a) pro rata dependant on qualifications and experience. Duties covered as part of our MMAPPPPSS Development Framework and your existing experience. Support with Training

  • 37.5 hours. 6 weeks annual leave + bank holidays
  • In this role you will work independently, making professional, autonomous decisions. You will be responsible for making any necessary referrals and requests for investigations. The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care. The role will involve independent prescribing within the scope of professional practice. The advanced nurse practitioner will support the GP team managing same day appointment requests from patients. The nurse practitioner will need to prioritise and triage the needs of patients accordingly, seeking GP input as appropriate if the problem lies outside their professional capability.   • To work as an autonomous practitioner
  • Attending to patients who wish to be seen by a health care professional
  • Diagnosing, treating and prescribing for patients presenting with undifferentiated and undiagnosed illness or injury
  • Making an assessment of their needs, based on highly developed nursing skills including physical assessment.
  • Seeing and advising people in respect of their continuing medical and nursing needs
  • Patients are to be seen in a timely manner enabling contractual requirements to be met
  • Prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. There will be a requirement to see extra or emergency patients
  • Developing, with the patient, a continuing management plan for the health of the patient and when appropriate to refer to other clinicians and agencies
  • Provide high quality nursing services to a defined practice population
  • Responsibility within the practice for the development and direction of the clinical and managerial aspects of nursing care, as well as an advanced clinical role
  • The role is flexible and demanding and may be expanded in accordance with practice requirements and the nurse practitioners own scope of practice.
