Full Job Description

The Diabetes Leeds clinical system brings together clinicians from primary care, the Community Diabetes Team and the Hospital Specialist Diabetes Service working together to deliver the best outcomes for people at risk of or living with diabetes.

Multidisciplinary working is at the heart of the teams core values and the appointment of this role builds on some ground-breaking work done in the last few years advancing the role of the pharmacists with the team.

Our Consultant Pharmacist for Diabetes has secured a Pre-doctoral Clinical and Practitioner Academic Fellowship award, and we are looking for a dynamic, passionate highly specialist diabetes pharmacist to work with us on secondment two days a week until May 2027. Please ensure you have secured the agreement of your employing organisation to second you for this length of time prior to applying for this post.

Working alongside the Consultant Pharmacist in Diabetes, this role will lead service development and the implementation of national guidance in medicines management for people across primary care and community services and identify research opportunities for collaborative working with academic partners.

Come and join our team to support others through change to maintain the delivery of high quality, compassion, proactive and safe diabetes care., You will work across the interfaces to deliver highly advanced clinical pharmacist services in diabetes across the Leeds Health and Care Partnership. This role will link, where appropriate, to the wider diabetes programmes of work across West Yorkshire Health and Care Partnership.

You will improve standards of pharmaceutical care for patients by strategically developing and leading the provision of highly advanced clinical practice promoting safe and evidenced-based practice and contributing to improved clinical outcomes and services to people living with diabetes, across the Leeds and West Yorkshire Health and Care Partnerships.

As the expert within medicines optimisation in a specialist area, you will act as a dynamic link between clinical practice and service development by creating new models for delivering patient care ensuring the best experience and outcomes for people from their medicines whenever and wherever their care is delivered. This role would provide a tiered approach to support, freeing up consultant pharmacist time.

You will be an opinion leader on pharmaceutical matters relating to diabetes care, including providing professional leadership and support to clinical pharmacists, including PCN and practice pharmacists working in primary care.
