Full Job Description

We are seeking a compassionate and dedicated Assistant Practitioner to join our Community Mental Health Team for Older Persons at the Sophie Centre, Julian Hospital, Norwich. In this role, you will support older adults experiencing mental health challenges, working alongside a multidisciplinary team to deliver high-quality, person-centered care., + Assist in the assessment, planning, and delivery of care to older persons
+ Work collaboratively with nurses, psychologists, and other healthcare professionals
+ Provide emotional support and practical assistance to service users and their families

Join our Community Adult Mental Health Service based in North Norfolk. We are currently going through an exciting transformation working closely with our Primary care colleagues and working towards aligning our team with our local GP practices.

The Trust has now introduced a DBS Update Service which is a contractual requirement. You need to subscribe when you have applied for your DBS clearance and there is a time limit to subscribe of 19 days after receipt of your DBS disclosure. Please ensure that this is completed within the set time scale.
This update service is an annual subscription at a cost of £13 to you. This will enable the Trust to have instant online access to your DBS record, with your consent, and so will remove the need for you to have to apply for this repeat check again.

+ Experience in mental health or elderly care is desirable
