Full Job Description

A great opportunity has arisen to work as an Assistant Practitioner (Band 4) within Bradford District Care Foundation Trust's Continence Service. We are a well-established team delivering expert care to individuals with bladder and bowel problems across the primary care setting and care homes. We are also responsible for delivering education across the district on continence care and catheterisation, the mechanism for this delivery is varied and responds to ever changing needs. We pride ourselves on our multidisciplinary approach to patient care and are very proud of the service that we deliver.

You will be responsible for undertaking continence assessments and reassessments, both in the community, and supporting staff and residents in care homes. The role will also include undertaking regular telephone clinics, supporting service users who require reassessment. You will be an effective member of the continence team, developing and maintaining effective channels of communication with other professionals and agencies.

Good communications skills, both verbal and written are essential for this role. You must be a flexible, caring, professional and have a passion for continence, with an ability to work in challenging situations that will involve problem solving and being supportive to patient needs.
