Full Job Description

The Special Allocation Scheme (SAS) was introduced as a Directed Enhanced Service in 2004, with the aim of providing a secure environment in which patients who have been violent or aggressive in their GP practice can receive general medical services. The Special Allocation Scheme is a Directed Enhanced Service to provide general medical services to patients who meet the criteria for inclusion into the scheme and cannot be used for any other circumstance., The post holder will have continuing responsibility for proactive assessment, monitoring and management of patients and ensuring care is current, evidence based and in line with practice standards. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. To deliver care with a Primary Care Service which includes primary mental health and substance misuse. The post holder is an experienced nurse, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. You will be expected to work collaboratively with key partner organisations, patient carer's, clinicians and other practitioners within the multi-disciplinary team in delivering services, supporting the delivery of policy and procedures. In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.
