Full Job Description

We are currently looking for an enthusiastic and creative Band 4 Assistant Practitioner to join our Cardiac Rehabilitation team.

Cardiac Rehabilitation Service is a multi-disciplinary team consisting of Specialist Physiotherapist and Cardiac Specialist Nurses and working collaboratively with other health professionals. We provide highly specialist assessment and intervention for adults with Cardiovascular conditions, Myocardial Infarction, Cardiac Procedures and Cardiac Failure as part of the National Association of Cardiac Rehabilitation pathway.

Cardiac Rehab aimed at motivating and educating service users to manage their condition more effectively and utilises both individual and group-based interventions. Close multi-disciplinary working within the team, is a key part of the service delivery and the successful candidate will be expected to play an active role in the delivery of the Cardiac Rehab Service in exercises class as well as offering individual and joint treatment sessions with other team members., Cardiac Rehab Physiotherapy team is part of the integrated community MSK Services working alongside with the Cardiac Rehab Specialist Nurses of East and North Herts Trust. The successful applicant will provide support to the cardiac rehab team and treatment to the cardiac rehab service users under the supervision of the Specialist Physiotherapist. You will have the opportunity to contribute to the development of the service.

It is expected that applicants will have excellent communication skills and experience in delivering interventions to both individuals and groups. As working with other professionals is an expected and integral part of the role, experience of, and a commitment to, multi-disciplinary working is essential. Regular supervision and CPD opportunities will be provided by the Specialist Cardiac Rehab Physiotherapist within the service.

You must hold a full valid driving licence and have access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
