Full Job Description
As a Care Coordinator, you'll be spending your time in the community conducting visits to review and assess the work of our Care Assistants. You will be supervising, supporting and guiding a team of carers on a daily basis and working closely with our Registered Manager to ensure smooth running of the team. The role will include being on a rota for being on-call responding to urgent matters relating to uninterrupted service delivery.
Working weekends and out of hours will attract enhanced hourly pay rates as will participating in the on-call rota., If you are someone who loves working with people and helping their development and always looking for ways to enhance quality of care, while working in an environment that's busy and fluid then this role is for you. Day-to-day, this includes:
+ Providing personal care,
+ Medication administration
+ Care planning for service users
+ Carrying out assessments on behalf of the team.
+ Actively participate in the professional development of self and others.
+ Participate in the on call service, outside of office opening hours.
You will need to have a full UK driving licence and will work in clients homes in the Harrogate area and at our Knaresborough based office., All designated tasks would be undertaken in accordance with agreed protocols drawn up by professionals working in the team. You will support people with a broad range of personal care tasks, such as, to wash and dress independently, encourage and motivate them with managing their nutrition and medicines and undertake health or therapeutic observations under the supervision of a Registered Practitioner.
The role includes the opportunity to develop clinical competencies such as wound care, catheter care, stoma care and peg feeding assistance, as well as enhanced competencies relating to Physiotherapy or Occupational Therapy, such as supporting the delivery of therapy plans, including Physiotherapy exercises, or assessing peoples equipment needs under the supervision of the relevant Registered Practitioner.
This role will work in partnership with all mainstream health and care services, to ensure the persons needs are meet in the most efficient and joined up way, and you will advocate for the person so that they only have to tell their story once. You will undertake appropriate risk assessments, and contribute to other professional assessments regarding both health and care needs, liaising with appropriate professionals to share and update relevant health and care records.
The role will support the Registered Practitioner in the provision of routine and fundamental health and social care support to people. The post holder will respond to service users personal health (both physical and mental health), and social care needs as well as wider independent living skills, such as social isolation, employment or other needs. Acting upon initial referral information but responding flexibly to the persons presenting needs.
This will include the following:
+ Supporting people to maintain a healthy and safe living environment
+ Supporting people to remain well-nourished and hydrated, including assistance in clinical health tasks, such as supporting PEG feeding, Stoma Care, or delivery of agreed speech and language therapy plans
+ Support people to maintain their personal appearance and self-esteem, including personal care
+ Assisting people to achieve rest and sleep by attending to comfort needs
+ Applying a person centred approach in the delivery of routine and fundamental health and care needs, delivering personalised approaches to support interventions
+ Safeguard the health and well-being of people in their care, reporting concerns promptly
+ Administer medication in accordance with policy, procedures and guidelines
+ Encourage access to universal services and mainstream community facilities, including voluntary sector services in their area
+ Maximise the persons ability to manage their own resources and circumstances, enabling them to maintain their independence for as long as possible, including managing risk in a positive manner
+ Ensure people manage long term health conditions, seeking advice from professionals as required to support people to live well with long term health conditions
+ Contribute to person centred support plans and reviews to achieve both health and social outcomes identified by the recipient of the service
+ Undertake continuous monitoring throughout the duration of the service, recording and reporting accurately
+ You may assist adults with maintaining their employment by providing support in the workplace and help promote natural supports in the workplace wherever possible
+ Provide appropriate personal care in a manner that promotes dignity and independence, assisting adults to maintain their personal hygiene and appearance
+ Deliver services to support wound care, monitoring and ordering stock as required, and report issues relating to skin integrity to professionals
+ Assess for, and in some circumstances deliver equipment and aids which support people to live independently. Carry out practical tasks e.g. fitting of simple adaptations in peoples
+ Have responsibility as case holder where equipment, adaptation or telecare only has been provided following an assessment, or to be an additional assessor for a caseload where other services are also provided, or allocated within the framework established by your line manager
+ Identify, fit, and demonstrate less complex equipment, adaptations and telecare to maximise independence.
