Full Job Description
We are seeking to recruit a highly experienced and motivated critical care Registered Nurse to join the Critical Care Outreach Team (CCOT) base-site at York Hospital, with potential to cross site cover in Scarborough Hospital. The Outreach Team is friendly and supportive with close links not only to our Intensive Care Unit but also a wide variety of multi-disciplinary teams across the hospital. Our team deliver a high standard of individualised care to deteriorating and acutely ill patients throughout the hospital 24 hours a day. The CCOT ensure a timely and effective response to patients, whilst supporting medical, nursing and allied health professional colleagues. Teaching and delivering education to ward teams is an essential part of the role together with leading on deteriorating patient quality improvement work across sites. In line with extensive work to improve hospital out of hours care, the CCOT have expanded their role to support collaborative team work across the Trust, and are instrumental with bleep filtering. We are looking for a highly motivated individual who is passionate about the care of patients and who will enjoy the challenge of working in this demanding but highly rewarding role.
The successful applicant must be able to demonstrate excellent clinical skills, including at least three years within the Critical Care environment as a Band 6, and sound evidence based decision making. They will carry out advanced assessment and care of deteriorating patients leading to early intervention and treatment. They must have excellent communication skills and be able to work autonomously.
- Work within frequently challenging clinical environments with high levels of physical and mental effort.
- Have knowledge of pathophysiology, anatomy and physiology in order to assess, plan, implement and evaluate treatment of the critically ill patient. Able to make sound clinical judgements involving complex situations which requires analysis and interpretation to decide on the best course of action/treatment.
- Undertake a comprehensive, advanced patient assessment based on history taking and clinical examination and implement a management/treatment plan.
- Competently and confidently care for the deteriorating/acutely ill adult utilising expert critical care skills and knowledge in a variety of clinical settings as part of the multi-disciplinary team.
- Provide direct patient care and support at the bedside until the patient is stabilised or transferred to an area with a higher level of care. Initiate therapeutic interventions for example to manage haemodynamic and respiratory instability.
