Full Job Description

An exciting opportunity is available for a band 5 nurse assessor within the team which is based at Laurence House in Catford. The role encompasses assessment of Nursing Home patients for eligibility for Funded Nursing Care and completion of the CHC checklist. This provides an opportunity to work with patients and relatives and also with the homes to promote quality and safety of care provision. There is a full induction and training programme with opportunity for development to a band 6 CHC assessor role. This can be discussed further at interview if candidates would like to pursue this pathway. Lewisham ICB reserves the right to close this post early if there is an overwhelming response. In line with most NHS organisations in London, we do not pay travelling expenses for interviewees. If you are offered a job, information will also be transferred into the national NHS Electronic Staff Record (ESR) system and other secure, internal NHS workforce systems in order to support and manage your recruitment and employment within the organisation., This post is part of the Continuing Healthcare Team, a multidisciplinary health and social care team which sits within Lewisham's Integrated Commissioning Team hosted by Lewisham Council. As a member of this team the post holder will:

  • Assess and determine whether individuals living at home or in supported care settings are potentially eligible/ remain eligible for NHS-Funded Nursing Care (FNC) or trigger (re)assessment for fully funded healthcare (CHC).
  • To review packages of care as may be required to ensure that the person's health needs are being well met.
  • Support the wider service quality intelligence network (health, social care and third sector) to advise on the quality of care provision and how it might be improved.

    NHS South East London Clinical Commissioning Group is the statutory body responsible for commissioning healthcare services to South East London's 1.9 million residents of the boroughs of Bexley, Bromley, Greenwich, Lambeth, Lewisham and Southwark.

    The Lewisham Continuing Healthcare (CHC) team provides a service to the population of Lewisham. It is an evolving team which has undergone continuous improvement as a result of development within the service.
