Full Job Description

As a CWD short breaks coordinator you will work with a number of children and families to understand their need for a short break, and work with them to ensure that they have the right provision in place to meet their needs and review the short breaks plan so that there is assurance that it is successfully meeting the child and family's need for a break and supporting the child's aspirations and preparation for adulthood.

You will be passionate about working with children who have SEND, committed to inclusion, and be creative and solution focused in supporting children to achieve their aspirations. You will work collaboratively and creatively with families, colleagues across education health and social care, partner and provider agencies, the voluntary and community sector, and other groups to assess the need for, design, implement and coordinate short break interventions for children and young people with SEND and their families and review the impact of this on the intended outcomes. You will be committed to Stockport's coproduction charter and the outcomes framework for children and families:
