We welcome applications from people who share our values and can help us deliver outstanding care in our local community., An exciting opportunity has arisen for a highly motivated, enthusiastic experienced registered nurse to join the Central Norwich Neighbourhood as a Band 7 Clinical Lead Nurse.
The successful candidate will provide professional and clinical leadership to staff working in the Central Norwich Neighbourhood and clinic-based services within the place portfolio. The successful candidate will support staff to achieve and ensure competencies are maintained in addition to ensuring that the Trust annual priorities and quality goals are embedded within the workforce.
The service operates between 08:00 and 18:30, the post holder will be required to work shifts within these times including bank holidays and weekends. The role rotates through the Operational Hub, coordinating shifts at weekends as the Senior Nurse for Norwich Place., Community nursing provides holistic patient centered care and nursing interventions to patients in their own homes, residential care and within nursing clinics within the Norwich Place portfolio, including catheter care, leg ulcer and wound care, diabetes and medicines management, and palliative and end of life care.
Working alongside the Clinical Operations Manager and as part of the wider leadership team in Norwich Place, the post holder will provide professional and clinical leadership to a friendly, supportive team of registered nurses, nursing associates and healthcare assistants, supporting staff to achieve and maintain competencies, promoting and contributing to the delivery of high standards of care to patients within their own homes, care homes and clinics.
The post holder will ensure NCHC's annual priorities, goals and behavior's are embedded within the team and place, contributing to staff wellbeing, and working alongside our key partners and stakeholders to ensure patients receive a seamless and integrated service., To provide professional leadership to own professional group
-To provide clinical leadership within the locality
-To work with, and deputise for, the Clinical Operational Manager as required
-To oversee and monitor plans of care for all complex patients on the caseload to achieve quality of life and independence where possible
-To work within the integrated team to prevent unnecessary admission to hospital and facilitate early discharge form hospital
-To work with all health care professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users
-To oversee that clinical supervision is occurring within the team and accurately recorded.
As part of operational leadership for the locality you will be required to:
-Rotate through the operational hub
-Be actively involved in the allocation of work for clinicians
-Ensure all staff are competent with mobile working
-Engage and lead on referral to discharge standard processes
-Work to a demand and capacity model which will reflect workload needs at -any given time
