Full Job Description
As the Clinical Nurse Specialist for the Older Peoples pathway and Memory Assessment and Treatment, within our Community Mental Health Service you will be responsible for:
Providing clinical leadership and practical support and advice to the team members.
Contributing clinical expertise to the team with regular clinical meetings and support the continual development of the service.
Planning, organising, undertaking, and documenting full bio-psychosocial assessment for patients referred to service, then designing, delivering, and evaluating a plan of care to meet the identified needs.
Initiating, monitoring, and reviewing prescribed medications under the supervision of the clinical supervisor in line with Trust policy.
Applicants should be an NMC registered Mental Health Nurse, with evidence of post registration training preferably in older people's mental health. Experience of coaching and mentoring is essential as this role will be responsible for supporting and developing the skills and knowledge of colleagues with the team.
Main duties of the job
o To assist managers and the Trust to develop and improve services and practice for patients and staff.
o Ensure that all staff are aware of and comply with NMC Code of Professional Conduct.
o To provide clinical leadership, guidance, support and coaching to staff, promoting effective change management and strengthening of other staffing leadership skills.
o Report to the Team Manager on the current state of staffing, seeking appropriate support as required.
o Contribute to the management of complex case work.
o To plan, organise, undertake and document full bio-psychosocial assessment for patients referred to service.
o Work closely with medical colleagues to determine appropriate treatment plans.
o To communicate diagnosis and treatment options to the patient and family.
o To provide advice, information and post-diagnostic counselling support to the patient and family.
o To initiate, monitor, and review prescribed medications under the supervision of the clinical supervisor in line with Trust policy.
