Full Job Description

An exciting opportunity has arisen for a Senior Community Mental Health Practitioner to join our Memory Team in Scarborough, based at Cross Lane Hospital. We are looking for a dynamic, positive and forward-thinking clinician with a passion for caring for people with cognitive impairment, their families and carers, and with a specialist interest in delivering high quality, effective and place-based Memory Services. This is an exciting time to join our team and locality - this role is an important part of a dynamic improvement review of how we deliver our memory services to broaden access and ensure delivery of specialist memory assessment in a timely manner. This is essential to enable timely diagnosis of dementia and associated needs and conditions, which supports access to early treatment and support, decision making and aids optimal management and life outcomes for those in need.

Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.

We welcome applications from dynamic, motivated and innovative Registered Mental Health Nurses, Occupational Therapists and Social Workers., The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. The ability to work independently, effectively and collaboratively within Multidisciplinary Teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.

Scarborough Memory Team is a small but lively, dynamic, and supportive team committed to collaboration and joint working. You will support the team's improvement activities for co-creation, aiming to deliver care that is highly personalised to each individual's needs and working closely with families, paid and unpaid carers. You will be encouraged and supported to bring your skills and experience to contribute to our team and locality's service development and continuous improvement and innovation endeavours.

Alongside the commitment from the Trust our team is small and friendly with a genuine sense of the importance of promoting staff well being. The Team has an identified and proactive well being champion, who aims to provide a range of opportunities for the team to think about how they stay well at work and in their personal lives.

The team is based in the same office space and works closely with the Scarborough Community Mental Health Team for older people, so you will benefit from extensive MDT support and input in your role.
